Omar Alderete was born on December 26, 1996, in Asunción, Paraguay. He began his football career with Cerro Porteño, where he played for their U20s at the 2016 U-20 Copa Libertadores.
In May 2016, Alderete made his professional debut for Cerro Porteño in a Paraguayan Primera División match against Rubio Ñu.

Alderete has played for several clubs throughout his career, including Cerro Porteño, Gimnasia y Esgrima, Huracán, Basel, Hertha BSC, Valencia, and Getafe.
